Physical Packet forms and School Insurance

Before any Student Athlete participates in School Athletics he/she must have completed the full physical packet in order to be eligible to play sports. Also, each Student Athlete must have health insurance.

Physical Packet includes:

• Pre-Participation Physical Evaluation (EL02)

• Consent and Release (EL03)

• HDSB field trip permission form

FHSAA REQUIRED COURSES

As per FHSAA Policy, all student-athletes and coaches are required to annually complete the following NFHS Learn courses before participation. All courses are FREE. In addition to an up-to-date FHSAA physical, any middle school student wishing to participate in a sport must complete the three required NFHS Learn safety courses (Concussion in Sports, Sudden Cardiac Arrest, and Heat Illness Prevention)
